Please help us to be ready to care for your student in the coming year. Here are some back to school tips for students with medical needs:<\/em><\/p><ol><li><strong><em>New medical needs or alerts?\u00a0<\/em><\/strong><em style=\"font-size: 16px\">Make sure the school has current medical information. Once school begins in August if you need to add anything to your student\u2019s health record, please send in a\u00a0written note (\u201cAttention: School Nurse\u201d)\u00a0to be placed with your student\u2019s file.<\/em><\/li><li><strong><em>Need to leave medication at school? <\/em><\/strong><em>Download any forms (right hand column) and bring them to your pediatrician.\u00a0A parent\u00a0and\u00a0medical provider must sign the forms for them to be valid. Only\u00a0<strong>new, unexpired<\/strong>\u00a0medications can be accepted. The Department of Health requires that all medications to be in their original container with a readable pharmacy\/consumer label. Please see our Medication Policy below for any further questions.<\/em><\/li><li><strong><em>Dropping off Supplies and Medication?\u00a0<u>The nurse will return to the clinic in August<\/u><\/em><\/strong>\u00a0<em>and will begin accepting medications and paperwork during that time. It is extremely beneficial to the nurse and your student to have the necessary paperwork and medication in the clinic <strong>before the first day of school.\u00a0<\/strong><\/em><\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bb00fb9 elementor-widget elementor-widget-text-editor\" data-id=\"bb00fb9\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p style=\"text-align: center\"><span style=\"font-size: 14pt;font-family: arial, helvetica, sans-serif\"><strong><span style=\"text-decoration: underline\">Important Information Regarding Medication<\/span><\/strong><\/span><\/p><ul><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>All medications <\/strong>(prescription and over-the-counter) must have a completed <strong>Medication Authorization Form<\/strong> (<a href=\"https:\/\/www.stjohns.k12.fl.us\/health\/wp-content\/uploads\/sites\/57\/2015\/04\/Medication-Authorization.pdf\">found here<\/a>) or <strong>Medical Management Form for a chronic condition <\/strong><\/span><\/li><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">All medications (prescription and over the counter) must have a completed Medication Authorization Form or Medical Management Form for a chronic condition.<\/span><\/li><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">All medications must be <strong>brought to school by a parent\/guardian<\/strong> and received in the <strong>original container<\/strong> with current Rx label (if prescribed). All over the counter medications must be <strong>unopened and in the original pack.<\/strong><\/span><\/li><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\"><strong>No medication will be given if presented at school in a plastic bag.<\/strong><\/span><\/li><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">Medication brought to school by a student will be kept in the clinic until a parent\/guardian comes to the school with the required paperwork. <strong>Without the required authorization, the student will not be given the medication during school hours.<\/strong><\/span><\/li><li style=\"text-align: left\"><span style=\"font-family: arial, helvetica, sans-serif;font-size: 12pt\">At the end of the school year, parents are responsible for picking up their child\u2019s medication.<\/span><\/li><\/ul><p style=\"text-align: center\"><span
